Common conversions

Always convert to the same unit before calculating.

mgmcg1000
kgg1000
LmL1000
lbkg÷ 2.2

Desired over have

Dose = (Desired ÷ Have) × Quantity. Example: 250 mg ordered, 500 mg/5 mL on hand gives (250 ÷ 500) × 5 = 2.5 mL.

IV rates

mL/h = total volume (mL) ÷ time (h).
gtt/min = volume (mL) × drop factor (gtt/mL) ÷ time (min). Round drops to the nearest whole drop.
